Project Funding Advance Notification
While Community Health Boards are not required to submit a formal application form to the Foundation in order to receive their annual CHB allocation, they must submit a "Project Funding Advance Notification Form", in writing, that outlines the nature and scope of their problem gambling project. The Advance Notification must include: the name of the Community Health Board, a contact name, address, email address, telephone number and fax number, and a brief description of the project with a projected timeline for completion of the initiative.
Upon receipt of the "Project Funding Advance Notification Form", a Foundation staff member will contact the CHB applicant to further discuss and strengthen the project idea, to ensure maximum impact and success.
Community Health Board problem gambling projects must be completed by March 31 of each year. Requests for extensions will be considered on an individual basis.
